Congratulations on completing your 30 hours of classroom education!

Please read the information below before submitting your form.

  1. You need an MV3001 form for the next step. This form will allow students 15 years or older to take their Permit Test at the DMV. (Please do not go to the DMV if the student is not 15 or older.)
  2. Along with the MV3001 form you need two forms of identification:
    (choose two of the following three)
    • Your Social Security Card (the DMV may accept your social security number, if you cannot find your card)
    • Your “Original” Birth Certificate (cannot be a photocopy)
    • Valid Passport (cannot be a photocopy)
  3. The student needs to pass a Permit Test. Due to Covid-19 we cannot conduct the testing at our locations. This test can be taken at any local DMV center and does not require an appointment.
  4. Please fill and submit the form below in order to request a copy of the MV3001 form.
  5. When we receive your request, we will provide your MV3001 form for download in your account - when this file is available, you will automatically be sent an email (sent to the email address associated with your account) notifying it is ready for your download, that will also contain the next steps you need to complete. These will include setting up a behind-the-wheel account and scheduling some lessons.

What Else Should I Know?

After Receiving Your Permit

Congratulations! If you already have your permit, you are now ready to drive.
Create a new account for behind-the-wheel lessons by following the steps below:
  1. Click “Register My Permit for Scheduling” in top right corner.
  2. Review “Step 1: Register (UNDER 18 Only)” and click “Register For Behind-the-Wheels”.
  3. Using your permit, complete the necessary information on the form.
  4. After your account has been verified, you will receive an email confirmation from our scheduling department. This may take up to two business days.
