United Driving School has temporarily suspended our voluntary Student Permit Testing (Face-to-Face) Program at our office locations. Some of you may know

Starting Monday, March 16th, students and/or parents may pick up the MV3001 Form at our office in Brookfield during normal business hours, or request to have it mailed directly to their home. This form will allow students 15 ½ years or older to take their Permit Test at the DMV. Students who do not wish to take their Permit Test at the DMV may defer their testing until a time when United is able to resume Permit Test sessions. We will continue to monitor the evolving situation and resume testing when it is advisable. If any changes arise, United Driving School will contact students and parents immediately. What to Bring to the Test

You do not need to bring anything to United Driving School in preparation for the test. Submitting this form is all you need to do. Just show up on time at the location below.

After You Pass the Test

We will give you all the necessary forms to take to the DMV to obtain your permit.

If you are already 15 1/2 and would like to go to the DMV right after your test, you will need to bring TWO forms of identification with you:
Your social security card PLUS
Your original birth certificate OR a valid passport PLUS
$35 permit issuance fee
